			<content:encoded><![CDATA[<p>Each tooth consists of a crown portion that is visible within the mouth, and a root portion which is under the gums, embedded within the jaw bones. The teeth are vital, meaning they each have their own blood and nerve supply for nutrition and for sensation. They receive this innervation through the root canals within the roots. The root canals then open up into a chamber within the tooth’s crown. These blood vessels and nerves within the tooth make up the pulp of the tooth, keeping the tooth vital.</p>
<p>When a tooth is infected with bacteria, it may reach the pulp chamber, spread rapidly and infect the pulp. This is when a filling can no longer help to prevent or eliminate the infection, and a treatment known as Root canal treatment (RCT) needs to be performed.</p>
<h2><strong>What is Root Canal Treatment?</strong></h2>
<p>Root canal treatment is also known as Endodontic treatment, meaning treatment inside the tooth. It is the removal of infected pulp, followed by disinfection and filling of the root canals system.</p>

<p>When a root canal treatment is performed, the tooth is no longer vital, as it does not have its blood supply and innervation anymore. The tooth can still persist and perform its function of helping in chewing food. However, to do so, it does need additional support with the help of a crown. Endodontic treatment is always followed by placing a crown upon the treated tooth to help maintain its functional integrity, as the remaining structure is brittle and can easily crack under masticatory(chewing) forces. The crown maintains the remaining tooth structure.</p>
<h2><strong>How is a Root Canal Treatment done?</strong></h2>
<p>First, the dentist or Endodontist will administer local anesthesia to block out pain. The infected root canal is then exposed by removal of the decayed tooth substance with drills, also known as hand pieces. This gives access to the root canals for the endodontic treatment, but it also involves destroying some amount of healthy tooth structure in the process. Then, very thin files are inserted through the tooth and into the root canals to empty out all blood vessels and nerves. These files are variable in length and width, giving them access to the narrow ends of root canals and to reach the tooth apices, where infection is most likely to spread through into the surrounding bone.</p>
<p>When the filing is completed, the next step is ‘irrigation’(washing out) the root canals with anti-septic solutions that are meant to decontaminate the root canals from any bacteria that remains. Sodium hypochlorite and chlorhexidine gluconate are commonly used solutions for irrigation. The root canal is sometimes temporarily filled with calcium hydroxide or other pack for a week, to aid healing the infection before a second visit. The paste is removed during the next visit and the root canals are filled with an inert material called ‘gutta-percha’.  This material is of a rubber- like consistency. A core material is placed above the gutta-percha to seal off the pulp chamber. The tooth is then prepared and shaped for the placement of a crown. This also involves removing healthy tooth structure but it is necessary for placing the crown effectively, to prevent the tooth from breakage.</p>
<p>Anti-biotics may be prescribed by the dentist to eliminate any further infection by remaining bacteria. Pain relief medications such as Ibuprofen can be taken to alleviate post- treatment pain and inflammation.</p>

			<content:encoded><![CDATA[<p>Tooth decay, in dental terminologies, is known as dental caries. It is a bacterial infection of a tooth or multiple teeth, and causes the formation of cavities, which can lead to further spread of infection and tooth loss if left untreated.</p>
<p>Caries is a dental problem most people have faced. Its earliest symptoms include pain in a particular tooth or associated region which is difficult to localize, sensitivity to hot, cold or sweet foods and pain on contact with the affected tooth.</p>

<h3>How does tooth decay begin?</h3>
<p>The normal bacterial commensals of the mouth are the main culprits involved in causing tooth decay, along with sugars from the diet. The most causative strain of bacteria in this category is <em>Streptococcus mutans</em>.<span id="more-11"></span></p>
<p>There is a natural bio-film that forms on the teeth within minutes after tooth brushing. This layer is known as dental plaque. The plaque itself is not what causes tooth decay as common misconception suggests. The bacteria in the mouth reside within dental plaque. When a diet containing sugars and acids is eaten, these are deposited on the teeth, contacting dental plaque. The bacteria then start to feed on the sugars and form acids, which begins the process of tooth decay.</p>
<p>Acids formed by bacterial breakdown of sugar begin demineralizing the tooth’s surface or crown, which is made of enamel. Enamel is known to be the hardest, most mineralized tissue found within the body, even more than bone. Following is the dentin, which is situated beneath the enamel and is responsible for supporting the brittle enamel. It too is highly mineralized but mostly has an organic tissue component, yet is still harder than bone. It forms the bulk of the tooth along with the root, and is the sensitive part of the tooth. Dentin is made up of dentinal tubules which contain dentinal fluid. This fluid is stimulated to flow to and from the enamel to pulp portion of the dentinal, bringing about sensation in the tooth from hot or cold foods. Within the dentin is the dental pulp chamber and root canal, containing the tooth’s nerves and blood vessels.</p>
<p>When <a href="http://en.wikipedia.org/wiki/Tooth_enamel">enamel</a> begins to demineralize, it becomes weaker and breaks down. Tooth decay then progresses to the dentin, which is when the tooth begins to hurt and feel sensitive. Once the tooth decay or caries has reached dentin, it can progress rapidly to the pulp chamber and down into the root canal from there, spreading the infection. This is a highly painful process, and needs to be treated as one may not even be able to chew properly due to the severe pain caries can cause.</p>
<h3>What causes tooth decay?</h3>
<p>Bad oral hygiene, allowing bacteria in dental plaque to persist and damage teeth is the main cause of tooth decay. Areas of the teeth which are difficult to clean even after brushing can still be subject to tooth decay, such as deep grooves and fissures on chewing surfaces and inter-dental spaces between teeth.</p>
<p>Other reasons for tooth decay, apart from high sugar foods, are exposure of the teeth to acids contained in carbonated soft drinks and foods high in acid content, such as citrus fruits. This can be controlled if water is used to rinse the mouth soon after consuming such beverages but people do not commonly do so, hence tooth decay can result.</p>
<p>Rarely, conditions such as acid-reflux disease and diabetes can lead to caries more easily. The regurgitation of acidic contents from the stomach can easily expose the teeth to acids, thus damage enamel. Diabetes leads to a high level of sugar in the blood and also in saliva, providing ample sugars for bacteria to thrive on and break down tooth structure. Moreover, people with a low salivary flow rate also suffer more tooth decay as there is less saliva to buffer the acidic environment in the mouth after consuming foods of the latter nature.</p>
<h3>Prevent and Treat tooth decay</h3>
<p>Maintaining good to almost perfect oral hygiene is the key to preventing tooth decay. Daily brushing, flossing and use of a mouthwash are ideal methods to maintain good oral hygiene.</p>
<p>If a small lesion has started to form on a tooth, it may even be reversed by simple use of fluoride tooth pastes and mouthwash. Larger cavities need to be filled and deeper cavities penetrating the pulp need to be treated with root canal treatment followed by placement of a crown on the affected tooth. In the most severe case, the tooth may need to be removed. Prevention is thus the key.</p>
